| Description | The purpose of this funding opportunity is to award one cooperative agreement recipient to serve as the Evaluation Provider (EP). This funding opportunity has a companion funding opportunity to fund an Implementation Technical Assistance Provider (ITAP). The two recipients will work collaboratively, but conduct distinct activities that support the overall initiative. The proposed initiative will be comprised of one Evaluation Provider (EP), one Implementation Technical Assistance Provider (ITAP) funded as a separate cooperative agreement, and up to eight (8) implementation sites sub-awarded by the ITAP.
